New Aftercare Program

Dear Youthdale Clients & Community,

We have listened to youth and families on how we can improve our services. As a result, we are very excited to announce that we have officially launched our Aftercare Program. The brief three month service is available to youth and their families transitioning from Live in Treatment services, ASU and TAU services as well as our Day Treatment program.

The program offers service navigation, revisiting strategies while transitioning back to the community and assistance in facilitating warm transfers. The program also includes a follow up call after a month’s time of discharge from aftercare services to check in and determine if other supports are needed. If youth and their caregivers are not sure about accessing the additional support, there is a three month window from discharge to still receive aftercare support.
